What is the Child Development and Care (CDC) Program?

The Child Development and Care (CDC) Program, offered by the Michigan Department of Health and Human Services (MDHHS), helps low- and moderate-income families pay for child care so parents can work, attend school, or participate in treatment programs. It provides a subsidy to help cover the cost of licensed or registered child care providers. This support helps families stay in the workforce, continue their education, and keep their children in safe, stable care.

How do I apply for the CDC Program?

You have a few options for applying:

  • Online: Apply quickly through the MI Bridges portal — this is the fastest and easiest option.

  • By mail or in person: Request a paper application (MDHHS-1171 and the CDC Supplement) from your local MDHHS office and submit it by mail or in person.

  • Provider Verification: Once you’ve chosen a child care provider, you’ll also need to fill out a Provider Verification form (DHS-4025) with your provider and submit it to MDHHS.

How long does it take to receive funds from the CDC Program?

Applications are typically processed within 30 calendar days after submitting a complete application with all required documentation. Once approved, your provider will receive an authorization notice and can begin billing for care.

What do the application steps look like?

Step 1: Apply online at MI Bridges or request a paper application.

Step 2: Submit your completed application along with required documents (see next section).

Step 3: Choose a licensed or license-exempt eligible provider and complete a Provider Verification form (DHS-4025).

Step 4: Submit the provider form to your local MDHHS office.

Step 5: Wait up to 30 days to receive a Notice of Case Action (approval/denial notice).

Step 6: If approved, your provider will get a CDC Provider Notice (DHS-198) and can begin billing through the I-Billing system.

Step 7: You’ll be responsible for any remaining balance not covered by the subsidy.

What documentation is required when I fill out my application?

Identity & Residency

  • State-issued photo ID (driver’s license, ID card, or passport)

  • Proof of address (utility bill, lease, or official mail)

Child Information

  • Child’s birth certificate or official hospital birth record

  • Social Security number or immigration documents (if applicable)

Income & Activity

  • Recent pay stubs, bank statements, or employer statement

  • Proof of participation in an approved activity (work, school, training program)

Other (if applicable)

  • Court order or doctor’s note if child is 13+ and needs constant care

  • Foster care verification documents

Common FAQs

1. Will the CDC Program cover all of my child care costs?
No — the program pays a partial subsidy based on your provider type and your child’s age. You’ll pay the difference between the subsidy amount and your provider’s rate.

2. What ages of children are eligible?
Children from birth up to age 12, or up to 18 if they have a documented disability requiring constant care.

3. What if my income, job, or school status changes?
You must report any changes within 10 days using MI Bridges or by contacting your local MDHHS office.

4. Can I have multiple child care providers?
Yes — you’ll need to submit a separate Provider Verification form for each one.

5. What if my provider isn’t licensed?
License-exempt relatives and in-home providers can participate but must complete the verification process and meet MDHHS standards.
